The front features a jockey and racehorse in the brand's signature red, black, and gold. The back lists all past winners since 1995, with 2006 left blank, as issued.

2006 was the first dead heat in Indiana Derby history. Star Dabbler's owners contested the result after their horse came out of the race injured, believing he had won — but the judges upheld the decision. The race moved to Horseshoe Indianapolis in Shelbyville in 2013, making these Hoosier Park glasses harder to come by.
